Identity and insurance

Your insurer will not process a request without these.

• Photo ID for the parent or guardian
• Insurance card — front and back
• Any secondary insurance card, front and back

Medical and diagnostic

The evaluation and the referral establish medical necessity, which is what your plan decides on. The rest help if you have them.

• The autism diagnostic evaluation
• A referral or prescription for ABA therapy

Forms we send you

These arrive by email once we have spoken. Nothing can be submitted until they are signed.

• The signed intake packet
• Consent and release forms

Not required to submit, but it saves a phone call.

• Your pediatrician’s name and phone number
• Contact details for any current therapy providers
• The days and times that genuinely work for your family

Four things that make this fast

• Send everything at once rather than piece by piece — an incomplete file is rejected and starts over.
• Photographs are fine, as long as all four corners are readable.
• Ask us if you are unsure whether something counts. We would rather answer than have it come back.
• Tell us early if a document is hard to get. We will gladly help chase it.
