Together We Care: A Family Resource Guide for BH Field Families

When your child has a disability, finding the right support can bring a long list of questions. Where do you begin? Which resources fit your family? How can you meet today's needs while preparing for the future?
You do not have to figure out every answer at once.
For BH Field families exploring community resources, Together We Care offers a starting point. The organization's family resource guide outlines three steps to help families share their needs, connect with resource-planning support, and begin building a plan.
Step 1: Complete the Family Intake Form
The first step is to complete the Together We Care Family Intake Form. This gives the team basic information about your family and your loved one's current needs.
That information helps the team prepare for appointments, identify potentially helpful services, and offer recommendations tailored to your situation.
Before you begin, take a moment to think about your biggest questions. Are you looking for respite care? Community connections? Help understanding funding opportunities? Sharing your priorities can make the conversation more useful.

Step 2: Schedule Your Free Annual Planning Appointment
The guide's next step is to schedule a free Together We Plan Annual Planning Appointment with Jillian Palmiotto.
This appointment focuses on your loved one's long-term future and an individualized family resource plan. According to the guide, topics may include:
- Long-term supports
- Housing options
- Employment opportunities
- Future funding strategies
- A sustainable support system
Your family does not need to have every future decision made before asking for help. Planning can begin with the questions you have today and develop as your loved one's needs change.

Appointment demand may be high. Check the scheduling calendar for current availability and reserve a time that works for your family.
Step 3: Schedule a Free Jump Start Connection
While you wait for your annual planning appointment, the guide recommends a free Jump Start Connection with Faye Yost, Together We Plan Coordinator.
This conversation gives your family an opportunity to identify immediate priorities and explore resources sooner. The guide explains that Faye can help families:
- Review current needs for Together We Care services
- Discuss respite care options
- Identify community resources near home
- Prioritize next steps while awaiting NOW Waiver approval, when applicable
- Ask questions about funding opportunities
Think of this as a place to begin organizing your next steps while your broader planning appointment is still ahead.

What About Katie Beckett?
Although the Together We Care resource guide includes Katie Beckett in its title, its instructions focus on connecting with Together We Care. It does not outline Katie Beckett eligibility requirements or the application process.
If Katie Beckett is one of your family's priorities, bring that question to your resource-planning conversation and ask where to find current program information and application guidance.
Start With One Manageable Step
Finding support can feel overwhelming, especially when you are already balancing appointments, school, work, and everyday family life. Begin with the intake form, schedule your planning appointment, and use the Jump Start Connection to explore your immediate questions.
You do not need a finished plan to take the first step.
